A formulary is the list of generic and brand-name prescription drugs covered by your plan. Drugs that do not appear on the formulary are not covered.
The formulary will also tell you:
- What tier a drug is in and how this affects what you pay for your prescriptions.

Talk to your doctor
Ask your doctor about prescribing medications on the formulary to make sure they are covered.
- If you’re taking a drug not on the formulary, you may be responsible for the entire cost of the medications.
- Some drugs on the formulary will cost more because they’re in a higher tier.
- Some drugs require preauthorization.
