D-SNP and eligibility
People who qualify for both Medicare and Medicaid are “dual-eligible.” If this is the case for you, you have a few different options for health care coverage.
You can join a D-SNP if you qualify for both Medicare and Medicaid. There are a few basic requirements. You must:
- Be a U.S. citizen or a lawful, permanent resident.
- Live in an area where D-SNP's are offered.
- Meet the financial requirements for your state's Medicaid program.
- Quality for Original medicare (Parts A and B)

We currently offer full state coverage for D-SNP.
New York

Pennsylvania
We currently offer D-SNP coverage in Western Pennsylvania and Southeastern Pennsylvania.
West Virginia
We do not currently offer D-SNP in West Virginia. Check back in July 2024.
