PITTSBURGH (March 29, 2023) – Titled “Bumps, bruises, warts, and wins – Inside the Social Determinants of Health (SDoH) Journey of an Integrated Finance and Delivery System and Their Initiative to Build a High Performing Social Care Network,” Highmark’s SDoH program leaders will provide an in-depth look into its successful High Performing Social Care Network pilot program and the results of that initiative to date, which created a $1 million value-based compensation program for 20 community benefit organizations surrounding its 14-hospital system at The RISE Summit on SDoH.
Amy Shannon, SDoH programs and products director at Highmark Health, David Zimmerman, SDoH care network program manager at Highmark Health, and Jamie Nee, director of care management at Allegheny Health Network, will represent the blended health organization at The RISE SDoH Summit, a national conference for cross-sectional thought leaders to discuss SDoH challenges and outcomes for the most vulnerable populations, on Monday, April 3 at the Fairmont Chicago, Millennium Park.
The presentation will also dive into Highmark’s multi-year journey to integrate SDoH screening referral into its approach to care for members and patients across its health system, and ongoing efforts, opportunities, and challenges to transfer this model to other network providers across the country.
“Building the High Performing Social Care Network is an important study of how we can better integrate and compensate social service organizations for the important role they play in providing value-based care,” Ms. Shannon said. “By honestly sharing Highmark’s successes and challenges in integrating social care into how we look at whole person health care, we hope to help move the conversation forward around how the health system can collectively enhance member and patient experience.”
Since the creation of Highmark’s SDoH program in 2018, it has integrated an SDoH screening tool into its approach to care, administering over 835,000 assessments with patients and members. Additionally, staff and community members have performed over 325,000 searches on Highmark’s “Community Support platform,” which connects patients and members with free or reduced-cost services in their local community such as food, housing, transportation, utility assistance, medical care, and job training.
